Quality and Safety Model for Quality Improvement

Children's Mercy Hospitals and Clinics uses comprehensive process — called TEAM — to guide quality improvement. 

Think of how to make things better

  • Identify a project
  • Ask "Is it quality improvement?"
  • Write mission statement or theory
  • Define goals and objectives
  • Determine outcome/measures desired

Establish the project

  • Identify the type of team (fast track, consultative, evidence-based practice, task force, etc.)
  • Select team members, team leader and facilitator
  • Define and communicate with oversight group
  • If it is a hospital-wide project take to quality council to determine the team
  • Develop data-collection tools
  • Collect initial data for the team and any background information needed by the team
  • Define terms and measurements
  • Use rating tools to rank evidence if basing on literature
  • Prepare team notebooks/arrange meetings for the team
  • Communicate kick off of the project with appropriate staff

Analyze the problem

  • Analyze symptoms of the problem and identify any root causes
  • Collect additional necessary data
  • Evaluate possible solutions to the problem
  • Design practice changes
  • Test theories by implementing a pilot or practice change and evaluate effectiveness of the change

Make changes and recommendations

  • Design the solution or decide to continue tested theory, modify, or redirect efforts
  • Present recommendations to oversight group for approval
  • Communicate recommendations and changes (plan for education)
  • Implement change
  • Evaluate effectiveness of the change
  • Ensure that gains are held by monitoring the process and outcomes
  • Provide periodic feedback to all appropriate staff
  • Replicate results and nominate new projects
  • Celebrate successes

Throughout the process, the team is expected to communicate their findings. TEAM report forms, executive summaries, storyboards, formal presentations and poster presentations are methods of summarizing findings.

Other communication includes educational releases, and quality council reports. This written communication allows for other employees to be educated on the findings of the team; provides a forum for input into the process by employees not participating on the team; and also continues to reinforce the theory and methodology of the quality-improvement process.
